Decode errors / corrupt packets

What we measure and how

Codec-specific decode-error signatures are matched in the merged video pass stderr. Each match (including `Last message repeated N times` accumulators) is counted as one error event. Reports total error count vs `decode_error_max_count` threshold from the spec.
